2021 Health Forms

Explanation of ALL forms found in the Enrollment Portal:

1. Form 1 (2 pages): This form can be completed online in your parent dashboard in bunk1.com. It gives us all the basic heath information about your child. Please give us as much information as you can on these forms. You can include immunization dates on this form as well.

2. Form 2: This form is MANDATORY.  In 2021, your child will not be admitted to camp unless this form is submitted. New York State Health Department will not allow us to accept ANY camper without this form. It MUST be signed by your child's Physician, Physician Assistant, or Nurse Practitioner. Our form is the only approved form by our Department of Health for our camp. Please do not send us a Sports Physical form or a general form given by your child's doctors office. We can only accept our Health Form 2 and it must be completed by your child's Physician, Physician Assistant, or Nurse Practitioner. 

3. Photo Permission: We have a wonderful photography program and our campers take photos all the time. We often like to post pictures on our Facebook page throughout the summer. We need you to fill out this form so we can use our photos. If you do not sign the form, we will not post pictures of your child.

4. Acknowledgment of Risk: This form is required by our Risk Management firm. It's lets us know that you understand the risks of sending your child to camp. Our activities are safe and our staff is well trained in Risk Management prior to camp opening. There is one for campers ages 8+ and one specifically for Cloverbuds. You only need to complete the one that pertains to your child's age.

5. Camper Code of Conduct: This needs to be reviewed and signed by you and your child. We are very proud the  4-H Clover Logo  and what it means to our camp. Head, Heart, Hands and Health. It is important to us that campers gain the life skills that we incorporate into all of our programs. We expect campers to honor the Clover through respectful, compassionate, cooperative and caring behaviors.

6. Meningococcal Form: This form is for those campers staying longer than 6 overnight days. (For Example: A camper who stays one week plus a weekend)


  All camper and health forms are found in the parent portal of our bunk1.com enrollment system . All enrollments must be completed online. Health Form 2 is mandatory and due to our office no later than 10 days before arrival to camp. Late registrations, please call the camp registrar for assistance in submitting forms. We will always accept last minute enrollments if there is space.
